Challenge

Treat-to-target approaches in UC similarly require objective monitoring and escalation when endoscopic or histologic targets are not met, but the real-world prevalence of this gap in UC patients across US academic and community practices had not been characterized with prospective registry data.

Solution

The TARGET-IBD registry was analyzed to characterize the proportion of UC patients failing to achieve treat-to-target goals and the frequency with which therapy was escalated in response, providing a real-world benchmark for guideline adherence in UC management.

Impact

Establishing the magnitude of the treat-to-target gap in real-world UC care provides actionable evidence for prescriber education and quality improvement programs, and reinforces the commercial and regulatory case for UC therapies that reliably deliver the endoscopic and histologic outcomes required by treat-to-target frameworks.
